The prostatitis indicator SPA generally refers to the ratio of free prostate-specific antigen (fPSA) to total prostate-specific antigen (tPSA) in prostate-specific antigen (PSA) examination. The normal reference interval may vary slightly depending on different medical institutions, but a general critical value above 0.25 may indicate a low risk of prostatitis.

If the fPSA/tPSA ratio is lower than 0.25, there may be a risk of prostatitis. Especially when the tPSA level is slightly elevated and the fPSA ratio is low, it may indicate that chronic inflammatory response leads to increased PSA release. For patients with acute bacterial prostatitis, PSA may be significantly elevated and usually returns to normal after anti-infective treatment.

If the SPA ratio is abnormal and accompanied by symptoms such as difficulty urinating, perineal pain, or fever, it is recommended to complete urine culture, prostate massage fluid examination, or ultrasound examination. In daily life, you should avoid sitting for long periods of time, eating spicy food, and excessive drinking. Regular ejaculation can help drain prostate secretions. Men over 50 years old or those with persistent PSA abnormalities must be reviewed regularly and checked for other prostate diseases.
