There is still hope for treatment after cervical cancer relapses, and the specific plan needs to be comprehensively formulated based on the site and scope of recurrence and the patient's physical condition. Recurrent cervical cancer can be intervened through surgical resection, radiotherapy, chemotherapy, targeted therapy and immunotherapy, and some patients can achieve long-term survival.
When there is local recurrence and no distant metastasis, surgical resection is the preferred option. Radical hysterectomy is suitable for patients with central recurrence, and can be combined with pelvic lymph node dissection after surgery. Wide parametrial resection is suitable for patients with lateral wall recurrence and needs to preserve bladder and rectal function. Some patients need to undergo pelvic organ resection, followed by postoperative radiotherapy to reduce the probability of recurrence. Radiation therapy is effective for inoperable local recurrence, and external beam radiation combined with brachytherapy can improve tumor control rates. Intensity-modulated radiotherapy technology can reduce damage to surrounding normal tissues and alleviate the risk of radiation enteritis and cystitis.
Patients with distant metastasis are mainly treated with systemic therapy. Platinum-based chemotherapy regimens such as paclitaxel combined with cisplatin can control disease progression. Anti-angiogenic drugs such as bevacizumab can inhibit tumor blood supply. The PD-1 inhibitor pembrolizumab is suitable for PD-L1-positive patients and kills cancer cells by activating the immune system. Under the multidisciplinary collaboration model, local radiotherapy can be used to relieve pain for bone metastases, and interventional embolization can be considered for liver metastases. Targeted drugs guided by genetic testing, such as PARP inhibitors, are effective in patients with specific genetic mutations.
Gynecological examination and human papillomavirus testing should be performed every 3 months after recurrence, and the treatment effect should be evaluated through imaging. Maintain a balanced diet, supplement high-quality protein, and exercise moderately to enhance immune function. If vaginal bleeding or pain worsens during treatment, please seek medical attention promptly. Psychological support can help relieve anxiety and depression, and the patient’s family should participate in the full care process. HPV vaccination can prevent other types of infection, but it cannot treat existing lesions.
