The structure of children's foreskin is mainly composed of the inner and outer plates of the foreskin, the foreskin opening, the coronal groove and the frenulum. It has the function of protecting the glans and maintaining local moisture.
1. Inner and outer plates of foreskin
The outer plate of the foreskin is the continuation of the skin covering the body of the penis, and the inner plate is connected to the mucosa on the surface of the glans, and the two meet at the foreskin opening. There is loose connective tissue between the inner and outer plates, which gives the foreskin a certain degree of stretchability. There are often physiological adhesions between the inner plate of the foreskin and the glans in childhood, and they gradually separate with age.
2. Preputial opening
The foreskin opening is the annular opening around the glans penis at the distal end of the foreskin, which is usually narrow in infants and young children. A normal foreskin should be able to retract moderately to expose part of the glans, but complete inability to retract or the presence of a fibrotic narrow ring may indicate pathological phimosis.
3. Coronary sulcus
The coronal sulcus is an annular depression at the junction of the glans penis and the body of the penis, forming a closed space when covered by the foreskin. There are a large number of sebaceous glands distributed in this area. The secreted substances and exfoliated epithelial cells together form smegma. Regular cleaning is required but excessive cleaning should be avoided to destroy the balance of local flora.
4. Lace structure
The foreskin frenulum is located on the ventral midline of the penis, connecting the inner plate of the foreskin and the lower part of the urethral opening, and is rich in nerve endings. A frenulum that is too short may limit foreskin retraction, but this is mostly a temporary phenomenon during childhood. Trauma or inflammation may cause scarring of the frenulum.
5. Vascular and innervation distribution
The foreskin tissue contains a rich capillary network and sensory nerve endings, especially in the frenulum area. This structure makes the foreskin sensitive to stimulation, and its good blood supply facilitates repair after injury. Special attention should be paid to protecting the major blood vessel and nerve bundles during surgery.
Parents should pay attention to the development of their children's foreskin, be gentle during daily cleaning, and avoid forcibly turning up the foreskin. If you find redness and swelling of the foreskin, difficulty in urinating, or recurrent infections, you should seek medical attention promptly, and the doctor will evaluate whether intervention is needed. Under normal circumstances, no special treatment is required, and most children will gradually improve with growth and development.
